Hope this helps others with this problem. But, this problem doesn't occur in other games using either a Xinput or Direct Input controller. I've played this game using a Dualshock 2 with a USB dongle, a Wii U Pro Controller with a Mayflower wireless USB adapter, a Steam Controller, and a Logitech F710 wireless controller and each one had this issue in FFXIV. SE needs to rewrite the code that handles Xinput and Direct Input controllers. This eliminates the problem, but it doesn't fix it.

I say this because this issue is not unique to Windows users, it also happens on PS3 and PS4. This fix is temporary until SE finally acknowledges it is a problem on their end. There is a bug in their game's USB controller enumeration code which causes this.
